4H Enrollment Forms-Cattaraugus

Join 4-H in Cattaraugus County by filling out all the downloadable forms below, enrollment fee of $5.00 per member or $15.00 per family and send them to:

Remember: You can only be a member of one county’s 4-H Program. You may fill out this forms each year, since they do not carry over from year to year.
