4H Enrollment Forms-Allegany
Enrollment Form- must be renewed every year, it does not carry over from year to year. Be sure to list any projects your interested in. This doesn’t mean each and every one needs to be completed, but if a clinic or workshop comes up, you will be sure to receive information on it if it’s listed under your projects!

There is a $5.00 enrollment fee per person or $15.00 for family of 3 or more. There is no fee for cloverbuds, which are 4-H members ages 5, 6, 7 and 8 (depending on date of birthday), contact the 4-H office if you have a question.
Medical Consent, Photo/Video Consent, Code of Conduct Form and the Acknowledgement of Risk Form, – must be renewed every year and code of conduct must be signed by the 4-H member.
Allegany County
Volunteer Code of Conduct - must be renewed every year and is signed by any and all adults working with 4-H.
CCE Volunteer Application – This includes a background check and should be renewed every 3 years.
Chaperone Guidelines – this is for County and State Fairs – Allegany County
